Forms of Payment

Payment for services is due in full at the time of service.  We accept most insurance plans and file all insurance claims. We ask that patients pay for the portion of services not covered by insurance at the time of service.

For your convenience, we accept cash and credit cards (VISA, MasterCard, and
Debit Cards). You may also make payment over the phone with a credit card. Please contact our billing department to discuss any special needs pertaining to your account prior to your appointment.

If you require special testing, procedures, or surgery, our personnel will make all arrangements, including obtaining referrals and pre-certification from your insurance
company.
